What Medicare does not cover
Medicare does not pay for the room-and-board or custodial care costs of assisted living or memory care. It's one of the most common and costly misunderstandings families run into. Medicare may cover short-term skilled nursing after a hospital stay, but not ongoing assisted living.
The main ways families actually pay
- Private pay: savings, retirement income, or proceeds from selling a home. This covers the majority of assisted living stays nationally.
- Long-term care insurance: if your loved one has a policy, check what it covers and how to file a claim — many families forget they have one until they look.
- VA Aid & Attendance benefit: an additional monthly benefit for eligible wartime veterans and surviving spouses who need help with daily living. It's underused — many eligible families never apply.
- California's Assisted Living Waiver (ALW) program: a Medi-Cal program that can help cover assisted living costs for eligible low-income seniors, in a limited number of participating communities. Availability and eligibility are specific — not every community participates.
- Selling or renting the family home: often the single largest funding source for families without long-term care insurance.
Ask any community directly whether they accept Medi-Cal's Assisted Living Waiver before assuming it applies — participation varies by community, and this changes which options are realistically on the table.
